The Air Carrier Certificate or Operating Certificate includes—
  1. (a)The certificate holder's name;
  2. (b)The location of the certificate holder's principal base of operations;
  3. (c)The certificate number;
  4. (d)The certificate's effective date; and
  5. (e)The name or the designator of the responsible Flight Standards office.
