We must obtain certain information from you to ensure we can properly bill, collect, deposit, and account for money you owe the United States. At a minimum, this information is:
  1. (a)Your full legal name;
  2. (b)Your correct mailing address; and
  3. (c)Your taxpayer identifying number.
