At a minimum, you must provide the utility with the following information when you request service:
  1. (a)Your full legal name or the legal name of the entity needing service;
  2. (b)Your taxpayer identification number;
  3. (c)Your billing address;
  4. (d)Your service address; and
  5. (e)Any additional information required by the utility.
