Existing contracting parties who receive Johnson-O'Malley funding must submit the following information in the annual report:
  1. (a)General information about the contractor;
  2. (b)General information about the number and names of the schools;
  3. (c)The number of eligible Indian students who were served using amounts allocated under the contract during the previous fiscal year;
  4. (d)An accounting of the amounts and purposes for which the contract funds were expended;
  5. (e)Information on the conduct of the program;
  6. (f)A quantitative evaluation of the effectiveness of the contract program in meeting the stated objectives contained in the educational plans; and
  7. (g)A complete accounting of actual receipts at the end of the fiscal year for which the contract funds were expended.
