The “Introduction and OSRP contents” section must provide:
  1. (a)Identification of the facility the OSRP covers, including its location and type;
  2. (b)A table of contents;
  3. (c)A record of changes made to the OSRP; and
  4. (d)A cross-reference table, if needed, because you are using an alternate format for your OSRP.
