An organization or individual may file a written signed complaint with an SEA. The complaint must be in writing and signed by the complainant, and include—
  1. (a)A statement that the SEA or an agency or consortium of agencies has violated a requirement of a Federal statute or regulation that applies to an applicable program; and
  2. (b)The facts on which the statement is based and the specific requirement allegedly violated.
