43 CFR §429.27

1. (a)An adequate description of the land, facilities, or waterbodies where the use will occur;
2. (b)A description of the specific use being authorized together with applicable restrictions or conditions that must be adhered to;
3. (c)The conditions under which the use authorization may be renewed, terminated, amended, assigned or transferred, and/or have the use fee adjusted; and
4. (d)Primary points of contact and other terms and conditions.
