The notice must include:
  1. (a)The tentative time, place, and nature of the hearing;
  2. (b)The legal authority and jurisdiction under which the hearing is being held;
  3. (c)The matters of fact and law to be asserted;
  4. (d)A description of the procedures for the conduct of the hearing;
  5. (e)The name, address, and telephone number of your representative and the NEA's representative; and
  6. (f)Such other matters as the ALJ deems appropriate.
