  1. (a)Documents filed with the ALJ must include:
    1. (1)An original; and
    2. (2)Two copies.
  2. (b)Every document filed in the proceeding must contain:
    1. (1)A title, for example, “motion to quash subpoena”;
    2. (2)A caption setting forth the title of the action; and
    3. (3)The case number assigned by the ALJ.
  3. (c)Every document must be signed by the filer, or his/her representative, and contain the address or telephone number of that person.
