The Commission publishes in the Federal Register upon establishment or revision a notice of the existence and character of the system of records, including for each system of records:
  1. (a)The name and location of the system;
  2. (b)The categories of individuals on whom records are maintained in the system;
  3. (c)The categories of records maintained in the system;
  4. (d)Each routine use of the records contained in the system, including the categories of users and the purposes of such use;
  5. (e)The policies and practices of the agency regarding storage, retrievability, access controls, retention, and disposal of the records;
  6. (f)The title and business address of the system manager;
  7. (g)The address of the agency office to which inquiries should be addressed and the addresses of locations at which the individual may inquire whether a system contains records pertaining to himself;
  8. (h)The agency procedures whereby an individual can be notified how access can be gained to any record pertaining to that individual contained in a system of records, and the procedure for correcting or contesting its contents; and
  9. (i)The categories of sources of records in the system.
