A notice of the proposed issuance, amendment, or repeal of a rule will include the following:
  1. (a)A statement of the time, nature and place of any public rulemaking proceeding to be held.
  2. (b)Reference to the authority under which the issuance, amendment or repeal of a rule is proposed.
  3. (c)Either the terms or substance of the proposed rule or a description of the subjects and issues involved.
  4. (d)The docket number assigned to the proceeding.
  5. (e)A statement of the time for filing comments and replies thereto.
