The notice required by § 840.3 shall include the following information:
  1. (a)Name and title of person reporting.
  2. (b)Name of railroad.
  3. (c)Location of accident (relate to nearest city).
  4. (d)Time and date of accident.
  5. (e)Description of accident.
  6. (f)Casualties:
    1. (1)Fatalities.
    2. (2)Injuries.
  7. (g)Property damage (estimate).
  8. (h)Name and telephone number of person from whom additional information may be obtained.
