Upon request, a duplicate certificate may be issued for a lost or destroyed official certificate.
  1. (a)Application. Requests for duplicate certificates shall be filed—
    1. (1)In writing;
    2. (2)By the applicant who requested the service covered by the lost or destroyed certificate; and
    3. (3)With the office that issued the initial certificate.
  2. (b)Certification requirements. The same information and statements, including permissive statements, that were shown on the lost or destroyed certificate shall be shown on the duplicate certificate. Duplicate certificates shall show: (1) The terms “Duplicate Original,” and the copies shall show “Duplicate Copy” and (2) a statement that the certificate was issued in lieu of a lost or destroyed certificate.
  3. (c)Issuance. Duplicate certificates shall be issued as promptly as possible and distributed according to § 868.70(b).
  4. (d)Limitations. Duplicate certificates shall not be issued for certificates that have been superseded.
