10. Documentation of periods when all qualified CISWI unit operators were unavailable for more than 8 hours but less than 2 weeks.
D. Emission Limitation or Operating Limit Deviation Report By August 1 of that year for data collected during the first half of the calendar year
By February 1 of the following year for data collected during the second half of the calendar year.
1. Dates and times of deviations
2. Averaged and recorded data for these dates.
3. Duration and causes for each deviation and the corrective actions taken.
4. Copy of operating limit monitoring data and any test reports.
5. Dates, times, and causes for monitor downtime incidents.
6. Whether each deviation occurred during a period of startup, shutdown, or malfunction.
