20 CFR §30.720

A request for a hearing shall be sent to the deciding official and shall contain:
  1. (a)A concise notice of the issues on which the provider desires to give evidence at the hearing;
  2. (b)Any request for the presentation of oral argument or evidence; and
  3. (c)Any request for a certification of questions concerning professional medical standards, medical ethics or medical regulation for an advisory opinion from a competent recognized professional organization or Federal, state or local regulatory body.
