An entity is eligible for direct payment of an authorized fee if the entity:
  1. (a)Has an Employer Identification Number;
  2. (b)Has registered with us in the manner we prescribe;
  3. (c)Has not been found ineligible for direct payment;
  4. (d)Designates and maintains an employee who is registered as a representative in the manner we prescribe as a point of contact to speak and act on the entity's behalf;
  5. (e)Accepts payment via electronic funds transfer; and
  6. (f)Conforms to our rules.
