A request for an “opinion letter” shall be in writing, signed by the person making the request, addressed to the Chair, Equal Employment Opportunity Commission, 131 M Street, NE., Washington, DC 20507 and shall contain:
  1. (a)The names and addresses of the person making the request and of other interested persons.
  2. (b)A statement of all known relevant facts.
  3. (c)A statement of reasons why the title VII interpretation or opinion should be issued.
