29 CFR §801.52

1. (a)Set forth the determination of the Administrator and the reason or reasons therefor;
2. (b)Set forth a description of each violation and the amount assessed for each violation;
3. (c)Set forth the right to request a hearing on such determination;
4. (d)Inform any affected person or persons that in the absence of a timely request for a hearing, the determination of the Administrator shall become final and unappealable; and
5. (e)Set forth the time and method for requesting a hearing, and the procedures relating thereto, as set forth in § 801.53 of this part.
