39 CFR §3055.92

1. (a)The report shall include a copy of each type of Customer Experience Measurement instrument, or any similar instrument that may supersede the Customer Experience Measurement instrument used in the preceding fiscal year.
2. (b)The report shall include information obtained from each type of Customer Experience Measurement instrument, or any similar instrument that may supersede the Customer Experience Measurement instrument including:
   1. (1)A description of the customer type targeted by the survey;
   2. (2)The number of surveys initiated and the number of surveys received; and
   3. (3)Where the question asked is subject to a multiple choice response, the number of responses received for each question, disaggregated by each of the possible responses.
