The OIG may impose a penalty of not more than—
  1. (a)$5,000 for each individual violation of § 1003.1100(a), (b), or (c).
  2. (b)$25,000 for each individual violation of § 1003.1100(d), (e), or (f) by a seller who is also the issuer of the policy; and
  3. (c)$15,000 for each individual violation of § 1003.1100(d), (e), or (f) by a seller who is not the issuer of the policy.
