This subpart sets forth—
  1. (a)The requirements for—
    1. (1)Entities that seek qualification as HMOs under title XIII of the PHS Act; and
    2. (2)HMOs that seek—
      1. (i)Qualification for their regional components; or
      2. (ii)Expansion of their service areas;
  2. (b)The procedures that CMS follows to make determinations; and
  3. (c)Other related provisions, including application fees.
