The UR plan must provide that written notice of any adverse final decision on the need for continued stay under § 456.436 (g) through (i) is sent to—
  1. (a)The ICF administrator;
  2. (b)The attending physician;
  3. (c)The qualified Intellectual Disability professional, if applicable;
  4. (d)The Medicaid agency;
  5. (e)The beneficiary; and
  6. (f)If possible, the next of kin or sponsor.
