Participation as an REH is limited to facilities that—
  1. (a)Meet the definition in § 485.502.
  2. (b)Have in effect a provider agreement as defined at § 489.3 of this chapter to provide services.
  3. (c)Meet the conditions of participation set out in this subpart.
